I do hereby grant consent for my name and image to be used in the filming/recording of videos, audio, and still photography to be used in a website, online, in printed materials and products, and in a video podcast for Good Dog Enterprises, LLC and waive my right to control or approve finished video or materials. All rights of any nature which may also arise from the videos, audio, pictures, or podcast are hereby granted, worldwide and in perpetuity, to Good Dog Enterprises, LLC. I hereby waive any rights to fees, royalties, or other compensation which may arise from participation in the videos, materials, products, and podcasts under the laws of the United States or any state thereof, with the exception of a product I provide to resell during the podcast, which will be covered under an addendum to this agreement. Good Dog Enterprises, LLC shall own the rights, title, and interest, including copyright, in the video, audio, recordings, and photographs. Good Dog Enterprises, LLC, may use and authorize others to use all or parts of the recordings and photographs. I grant full permission for the use of my name, likeness, performance, voice, and biography in the videos, audio, and still photography for the production of a video podcast series on dog related subjects, products, printed materials and in publicizing, advertising, or promoting of videos, audio and materials in any medium, including audio tape, film, internet, print media, radio, television, and social media.
